 Post subject: P0138 Code After Replacing Rear Oxygen Sensor

I have a 1997 Plymouth Voyager with a 3.0 motor. I had a P0138 code so I changed the rear sensor last night. The check engine light went off for about 10 miles and then came back on with the same code. Anyone have any ideas what I should do next? Thanks in advance for any help.

I'd imagine then the circuit to the 02 is no good or the 02 you put in is incorrect.
