How can it be BOTH 401 (Insufficient EGR Flow) and 402 (Excessive EGR Flow)? What can cause this?

Replace the DPFE with a new one from the Dealer. Aftermarket DPFEs don't last for very long.

Because the DPFE is bad, the voltage sometimes is high and other times it does not move. Replace it with a dealer part.
